The first step to understanding how to complete treatment plans is to fully grasp the terminology. This page will attempt to define what exactly is meant by the terms, problems/symptoms, goals and objectives.

Included are sample lists of some of the most common problems encountered by our patients. The sample lists are intended only to help guide the thought process in formulating treatment plans. They are neither comprehensive nor specific enough. Each plan should be tailored to the individual's problems and needs. |
MCDR’s stated goal is to help ensure that every patient’s treatment is clinically appropriate and cost efficient. One of the means by which we attempt to achieve this goal is clinical monitoring through psychiatric treatment plans. Treatment plans should serve as a guide to the treatment process such that at every stage of intervention, providers are forced to think critically and analytically. They help the clinicians to focus on the problems that brought the patient into the treatment in the first place. They also help to clarify the treatment process to concrete, measurable outcomes. They should be specific, focused and clear. Despite these clarifications, we at MCDR find many of our clinicians still struggling with completing the required treatment plan forms. Therefore, included in these web pages are guidelines to assist in this process as well as examples for some of the most common problems. Please click on the links above.
