PHOBIA Sample Treatment Plan

  Problems:
       
Persistent excessive or unreasonable fear of specific object or situations Avoidance, fear or anxious anticipation of encountering the phobic stimulus interferes significantly with the person’s daily routine, occupational functioning or social life or if the person is markedly distressed about having the phobia
Concerns about losing control, panicking and fainting when exposed to the feared object    
       
       
  Goals:    
       
Develop reasonable exposure strategies Reduce the avoidance behavior and anxious anticipation of the phobic stimuli
Reduce fears so that patient can resume normal daily functioning both at home and at work Achieve greater understanding and insight into maladaptive cognitive and behavioral processes that only fuels the fears
       
  Objectives:    
       
Patient will cooperate with psychiatric and psychological evaluations and follow through with recommendations Patient will verbalize positive, healthy and rational self-talk that reduces fear and allows encounter with avoided stimuli
Patient will comply with medication prescribed to alleviate anxiety Patient will develop cognitive and behavioral strategies to reduce irrational fears
Patient will construct a hierarchy of situations that increasingly evoke anxiety Patient will develop and utilize relaxation strategies to reduce symptoms
Patient will cooperate with systematic desensitization to the anxiety provoking stimulus or situations
