PAIN DISORDER Sample Treatment Plan

  Problems:
       
Evidence of significant distress or impairment in social, occupational or other important areas of functioning Pain becoming a major focus of ones life, often with excessive use of mediation and concurrent relational problems
Inability to work or attend school as result of pain Pain leading to increasing inactivity and social isolation
Frequent use of the health care system Onset of additional psychological problems and significant reduction in physical endurance leading to fatigue and greater pain
       
  Goals:    
       
Reduce subjective experience and increase tolerance of pain Reduce frequency of health care utilization
Achieve manageable pain to return to a more productive healthier living Develop greater control over daily living
       
  Objectives:    
       
Patient will complete a thorough medical and psychiatric examination Patient will increase awareness of mind-body connections
Patient will identify how pain has made an impact on daily activities Patient will develop various relaxation techniques, exercise routines, and other meditation to help ease pain
Patient will follow through on recommendations made by treating physicians and therapeutic interventions developed with therapy Patient will inquire into appropriateness of pain management programs and follow through on recommendations
Patient will identify triggers for pain Patient will identify dysfunctional attitudes about pain that only reinforces the problems
Patient will verbalize and implement stress-coping steps
