OBSESSIVE COMPULSIVE Sample Treatment Plan

  Problems:
       
Persistent ideas, thoughts, impulses or images that are experienced as intrusive and inappropriate and that cause marked anxiety or distress  Repeated thoughts about contamination, doubt, aggressive or horrific impulses, need for order or sexual imagery
Repetitive behavior to prevent to reduce anxiety or distress Behavior demonstrating significant interference with the individual’s normal routine, occupational functioning or usual social activities or relationships with others
Attempt at ignoring, suppressing or neutralizing obsessions with some other thought or action    
  Goals:    
Reduce time spent on obsessions and compulsive behavior Resume as much of the normal daily functioning free from obsessions and compulsions

Identify and reduce stressors that may have exacerbated the OCD    
       
  Objectives:    
       
Patient will comply with psychological and psychiatric evaluations Patient will comply with exposure and response prevention strategies
Patient will comply with recommendations of the psychiatrists including taking medications as prescribed Patient will verbalize increased insight into his/her maladaptive thoughts and behaviors
Patient will implement cognitive strategies to interrupt obsessions Patient will reduce the amount of time spent on obsessions and compulsive behaviors
