DEPRESSION Sample Treatment Plan

  Problems:
       
Loss of interest or pleasure in nearly all activities Significant distress or impairment in social, occupational or other important area of functioning
Change in appetite or weight, sleep, or psychomotor activity Presence of irritability or angry outbursts
Feelings of worthlessness or guilt Social isolation or disinterest
Difficulty thinking, concentrating or making decisions Decreased energy, tiredness and fatigue
Recurrent thoughts of death or suicidal ideations, plans or attempts Impaired ability to think, concentrate or make decisions
  Goals:    
       
Improve depressive symptoms such that patient is able to return to previous level of functioning Develop a more accurate cognitive about self and surroundings and healthier behavioral pattern of adaptation
       
  Objectives:    
       
Patient will demonstrate elevation of mood as manifested by increased range of affect and stabilization of appetite, sleep, energy and activity Patient will replace excessively negative thought and talk with more realistic cognition and speech
Patient will specify what in the past or present life may have contributed to depression Patient will verbalize hopefulness about the future
Patient will take medication as prescribed by a physician Patient will develop and utilize behavioral strategies to overcome depression
Patient will verbalize any history of suicidal ideations and any current urges Patient will identify and verbalize any unresolved grief or conflict issues that may be contributing to depression
